I am aware that participating in this sport can be a dangerous activity with risk of injury. I understand that my childs participation could lead to injuries such as, but not limited to, sprains and strains of muscles and joints, serious neck and spinal injuries, injury or impairment to other aspects of my body, general health and well-being, and in severe cases, even death. I agree to allow the supervisors and athletic trainers to treat my child in cases of emergency. Because of the potential dangers in participating, I recognize the importance of following the supervisors� instructions and adhering to all rules set forth. I hereby voluntarily release the right to hold Lowcountry Lacrosse and their staff from any and all responsibility should injuries, losses or damages of any kind be caused by my childs participation. I agree to be personally responsible for any damages to any property caused by my childs negligence or failure to follow instructions, rules and regulations. My typed signature demonstrates my understanding of all the previous information and my willingness to comply with these guidelines.
Full refunds will only be made prior to the beginning of the program if your child has a medical reason with a written note from their doctor. If your child becomes unable to participate after the season has started due to medical reasoning verified by a doctor, a refund will be granted on a pro-rated basis. A 75% refund will be given for all cancellations made with more than 30 days notice to the start of the program. A 50% refund will be granted if we are notified within 15-30 days of the start of the program. Once the program is less than 14 days from its commencement, NO refunds can be given.
